Hyde Hope Haven of North Carolina believes that accountability begins with documentation.
Individuals and families sometimes experience problems involving public agencies, service systems, courts, schools, law enforcement, child or adult protective services, guardianship, foster care, or other organizations entrusted with serving the community.
A single experience may appear isolated. Multiple documented experiences can reveal patterns. Which have begun to appear.
HHH's Community Concern process provides individuals with a way to document concerns involving issues such as lack of communication, failure to investigate, bias, retaliation, due process concerns, failure to protect, delayed services, adult neglect, elder exploitation, and other troubling practices or experiences. Those are the categories contained in your Community Concern form.
What We Do With This Information:
Information submitted through the Community Concern Form may help Hyde Hope Haven:
*Identify recurring concerns or patterns
*Better understand problems affecting individuals and families
*Support advocacy and community education
*Identify situations that may warrant additional resources or referrals
*Document concerns involving particular practices, offices, or systems
*Inform appropriate accountability and oversight efforts
